Your nominee may be required to submit the following documents to claim death benefits -

a. Death certificate (self-attested copy), Claimant statement form, KYC document of the beneficiary (self-attested copy), and bank details of the beneficiary.
b. Additional Requirements For Claims Within 3 Years: Self-attested copies of hospital or treatment records (if any), Original policy document, medical attendant's certificate (if any), and employer's certificate (if applicable).
c. Additional Requirements For Accidental Or Unnatural Death Claims: FIR and final police closure report, valid vehicle insurance document of the vehicle (if death was because of a road traffic accident), post mortem report, driving license (in case of death while driving), police inquest report/inquest panchnama, newspaper cutting (if any).

If the insured survives, they will need -
A. Properly filled out claim form or the payout form.
B. Original policy document.
